Three ways to send

For referring providers

Send a referral the way your office already works. One team reads every referral, calls the patient and tells you when care starts.

  • Secure link

Open our referral page and send it from your office, the way you send other referrals.

  • Fax

Fax your office's usual referral form to (507) 555-0107. We confirm receipt to your office.

  • Phone

Call the referral line at (507) 555-0106, Monday through Friday, 7 a.m. to 5 p.m. A person answers.

What happens after you send it

  1. 01

    Your office sends the referral

By secure link, by fax or by a call to the referral line. Send what your office usually sends.

  2. 02

    We call the patient

A scheduler offers times at the site closest to home or work and tells the patient what to bring.

  3. 03

    A therapist evaluates

The first visit is an evaluation. The therapist explains what they find and agrees on a plan of care with the patient.

  4. 04

    The referring office hears back

We send a note after the evaluation and when care ends, and we call sooner if something needs a doctor's attention.

Choosing a site

Any of our five sites can take a referral. If the referral does not say which, we offer the patient the one nearest home or work. If you prefer a site, write it on the referral.

  • Downtown Rochester and Southeast Rochester

  • Northwest Rochester

  • Byron and Stewartville

Questions from offices

Do I need a referral?

In Minnesota you can start physical therapy without a referral. Your plan or your employer may still ask for one. Most of our patients come with a referral from a physician's office, and we send that office a note. Call with your plan card and we will check before you book.

Can a referral go to any of your sites?

Yes. Every site reads referrals from the same line. If the referral does not name a site, we offer the patient the one nearest home or work, and we can move a patient to another site later.

What about a work or auto injury?

Tell us who your adjuster is when you call. Your state decides who may direct your care, and we explain what that means for you.

Talk to the referral desk

A person answers the referral line on weekdays, and the fax and secure link are read every working day.
